Timely Reminder: Schedule Student Physicals Now for 23-24 School Year; Due in September

Our thanks to our North Colonie CSD school nurses for their work and this timely reminder:

Be sure to reach out now and make your doctor appointments; the summer appointments fill up quickly for most providers.

Physical examination reports for grades K, 1, 3, 5 ,7, 9 and 11 and any new student entering the district for the first time.
All required physicals (grade levels listed above) are due at the start of each school year. Please send in the more recent physical exam report. These exams must have been completed after September 6th, 2022 in order to be valid.  Anything dated prior to that will not meet the requirements and mandate.  If you do not yet have a physical scheduled with your primary care doctor, please reach out now or over the summer as appointments fill up quickly.  Students playing sports will need reports submitted prior to trying out. 

Attention Parents of Students Entering 6th Grade and Higher.
Students entering 6th grade:   in order to begin school in September all entering 6th graders must have on file in the Health Office by September 1st the documentation of receiving their Tdap vaccination.

Students entering 7th and 12th grade:  in order to begin school in September all entering 7th and 12th graders must have on file in the Health Office by September 1st the documentation of receiving the Meningitis vaccination. 

Students playing fall sports:  If your student will be playing a fall sport, they will also need an updated physical before they can even try out.  Most sports tryouts  begin mid -August to early September. Check the North Colonie athletics page for additional information and deadlines.
