North Colonie Athletic Department will be providing FREE SPORTS Physicals for all high school students and only students who are entering 7th and 8th grade. The physicals will be performed by our school doctors.
REQUIRED: NYS SCHOOL HEALTH EXAMINATION FORM: you only need to fill out the top.

LOCATION: High School H Gym 3:00 p.m. to 5:00 p.m. (middle school students do not need to dismiss early)
LOGISTICS: Students will arrive at the High School H gym. They do not need to be accompanied by a parent or guardian. Check in at the desk. Wait for a nurse to obtain height, weight, and blood pressure. The student will be escorted into the H Gym Locker rooms and be seen by a Doctor.
