This form does not authorize access to North Colonie Central School District’s online software applications, i.e. IEP Direct, Infinite Campus, etc.

I have read the attached Information Technology and Network Use guidelines and I agree to all district regulations and the following:

1. Intellectual materials generated as part of work requirements, and/or stored on any system provided by the district, may not be provided for sale or profit with or outside of the North Colonie Central School District.
2. All information and services available on the Internet and North Colonie Information Network are placed there for informational purposes. This form does not authorize access to North Colonie Central School District’s online software applications, i.e. IEP Direct, Infinite Campus, etc.
3. The North Colonie Central School District does not warrant the function of the North Colonie Information Network or any component accessible through the North Colonie Information Network to meet any requirements that are beyond those established by the District, or that the North Colonie Information Network will be error free or uninterrupted. District staff is not liable for any damages incurred in connection with the use, operation, or one’s inability to access the North Colonie Information Network.
4. In consideration for using the Information Network and having access to public networks, I hereby release the North Colonie Central School District and its officers, employees and agents from any claims and damages arising from my use or inability to access the North Colonie Information Network.
5. I will also abide by such rules and regulations as adopted by the North Colonie Central School District including but not limited to Policy 4550 and 4550 Regulations, as revised as of May, 2008. I also understand that any violation of such rules is unethical and may constitute a criminal offense. Should I commit any violation, my access privileges may be revoked and disciplinary action taken.
