Albany DOH Vaccine Clinic – Wed., 11/17 at Blue Creek Elementary School

The Albany County Department of Health will be administering FREE COVID-19 Vaccine for those 5+ Years Old on Wednesday, 11/17

You MUST make an appointment.  No walk-ins will be accepted.   For general information about the COVID-19 vaccine, please visit the CDC Fact Page

General Clinic Reminders:

  • Arrive wearing a mask and practice social distancing regardless of vaccination status.
  • All persons aged 5+ years old are eligible to receive the vaccine.
  • All persons 15 and under will be required to attend the clinic with a parent or guardian.
  • All persons 16 and 17 arriving without a parent or guardian will be required to arrive with a signed

 NYS COVID-19 immunization Screening and Consent Form.

  • NO insurance, social security number or medical paperwork needed.
  • NO fee or cost for the vaccine – it is FREE.

COVID-19 Vaccine Formulas and Doses Offered Here:   

  • Pediatric Pfizer 1st and 2nd doses for children 5-11 years old.
    • The Pfizer COVID-19 vaccine is a two-part vaccine.  Individuals receiving their 1st dose on November 17, 2021, will be scheduled for a 2nd dose December 8, 2021.

  • Pfizer for 12+ Years Old
    • 1st and 2nd for individuals 12+ years old.   The Pfizer COVID-19 vaccine is a two-part vaccine.  Individuals receiving their 1st dose on November 17, 2021, will be scheduled for a 2nd dose December 8, 2021.
    • 3rd doses for individuals 12+ years old meeting eligibility criteria.
    • Booster Shots for individuals 18+ years old meeting eligibility criteria.

  • Moderna for 18+ Years Old
    • 1st and 2nd for individuals 18+ years old.  The Moderna COVID-19 vaccine is a two-part vaccine.  Individuals receiving their 1st dose on November 17, 2021, will be scheduled for a 2nd dose on December 15, 2021.
    • 3rd doses for individuals meeting eligibility criteria and 18+ years old.
    • Booster Shots for individuals 18+ years old meeting eligibility criteria.

Booster Shots:  To determine if you are eligible for a booster shot, please visit:  https://covid19vaccine.health.ny.gov/booster-doses .  If eligible, you must arrive with proof of vaccination – your vaccine card or Excelsior Pass Plus on your phone.
