Teen Cyber Census Informed Consent for two online surveys

We are pleased to share with you that Shaker High School is participating in the Teen Cyber Census, administered by the Siena College Research Institute (SRI) in collaboration with AT&T and the Tyler Clementi Foundation.   We are very excited to have the opportunity to participate in this survey because it allows us to be proactive in exploring our children’s Internet habits and open a dialogue with them about appropriate behavior and safety.

We are asking permission for your child to take part in a study offered to all North Colonie Central School District students in grades 7-12. You may choose to participate in this study as a parent/guardian. This letter will give you information about the study so you can decide if you want your child to participate. A valid email address for you and a valid email address for your child will be required for you and/or your child to enroll in the study.

We are especially excited about the special prizes classes can earn!  The junior high could get $350 for their end of the year awards programs just by achieving a 50 percent survey response rate.  In the high school, there is a competition between grades 9-12:

First Prize: $400 – to the class that achieves the highest number of registrations
Second Prize: $150 – to the class with the second highest number of registrations
Third Prize: $100 – to the class with the third highest number of registrations
