Registration for all new students occurs at the
Central Registrar's Office, located at
91 Fiddlers Lane
Latham, NY 12110. (518/785-8591, ext. 3130). Registration hours are from
8 a.m. to 4 p.m., Monday through Friday, BY APPOINTMENT
ONLY. Please call to request a registration packet. When this
packet is complete, please call again to set up an appointment.
Items required at time of registration:
One of the following:

Deed/mortgage (If you are not the owner of
the house, but live with the owner, such as a grandparent of the
child, please call the Registrar Office at 785-8591, ext. 3130
for additional instructions).

Lease (student's name must appear on lease
or on lease application)
Also needed:

Bank account and/or Credit Union Statement

Paycheck
-
Phone and/or Cable Bill
-
Automobile and/or Home Insurance Policy

National Grid Bill (Niagara Mohawk) Note: All bills must be dated from the previous month.
Driver's licenses and tax bills are not acceptable.

Original birth certificate. Copies are not
acceptable.

Immunization record (official record signed
by a physician). Copy of physical is not required at time of
registration, but latest immunization record is required.

Copy of custody papers, if relevant.

Additional information:
A health physical must be provided in order to
attend school. Physicals must have been performed within 12 months
of the start of school. If a physical has not been performed, you
are allowed 30 days from the start of school to obtain one. The
green health form is provided in your registration packet for your
convenience. Please keep in mind, however, that the latest
immunization records are absolutely required in order to register.
Kindergarten Registration
To be eligible for kindergarten at the opening of school of any
year, a child must be five years of age on or before December 1.
