The Registrar’s Office will be closed from
Wednesday, May 22 – Wednesday, May 29, 2013. The Registrar will
return to the office on Thursday, May 30th. If you have any urgent
registration/residency matters during that time, please contact Dr.

Registration for all new students occurs at the
Central Registrar's Office, located at
91 Fiddlers Lane
Latham, NY 12110. (518/785-8591, ext. 3130). Registration hours are from
7:30 a.m. to 3:30 p.m., Monday through Friday, BY APPOINTMENT
ONLY. Please call to request a registration packet. When this
packet is complete, please call again to set up an appointment.
Kindergarten Registration
To be eligible for kindergarten at the opening of school of any
year, a child must be five years of age on or before December 1. If
your child is between the ages of birth and five, please call
785-8591 ext. 3130.
Registration takes place beginning in December for the following
school year.
For September 2013 Kindergarten enrollment, your child must turn
five (5) by December 1, 2013.
Items required at time of registration:
One of the following:
Deed or Mortgage
If you are not the owner of the house, but live
with the owner - such as a grandparent of the child - please call
785-8591 ext. 3130 for further instructions. You will need to
provide additional documentation.
Lease
If you rent an apartment or house, your child's
name must appear on the lease or lease application. If the child's
name is not listed, then, in addition to the lease, you must
also bring a letter from the landlord stating that the child lives
at that address. The letter must include the landlord's name and
phone number.
Contract to Build/Buy a Home
For September entrance only.
In addition to the above, two (2) more proofs of residency
are required.
Any two of the following are acceptable:

Bank account and/or credit union statement
-
Paycheck
-
Automobile and/or homeowners/renters
insurance policy
-
Automobile registration (NOTE: A driver's
license is not acceptable)

Telephone, cable and/or utility bills
Documents must be current and original -
they will be copied and returned to you immediately. Print-outs from
online accounts will be accepted if they show the name and address
of the resident and have a current date.

Parent/Guardian government-issued photo
identification (such as a driver's license or passport)

Original birth certificate or certified copy (NOTE: A "Certification of Birth" document is
not acceptable)

Immunization record (official record signed
by physician)
-
Custody papers (NOTE: Please bring most
recent court order. It must clearly state who has
physical/residential custody of the child and must have judge's
signature)
-
All forms included in the registration
packet - filled out as completely as possible.
If relevant, additional documentation needed for
school information

IEP (Individualized Education Plan) from
previous school district

Section 504 documentation from previous
school district

Last report card for placement purposed
Additional Information
A health physical must be provided in order to
attend school. The exam must have been performed within the 12
months prior to the child's start date in our school district. If a
physical has not been performed, you are allowed 30 days from the
start of school to obtain one. The Health Certificate/Appraisal
Form is provided for your doctor's use. Please remember,
however, that the most current immunization records are absolutely
required in order to register (see above).
